Problem-Attic makes a large body of questions available for free to the education community, for personalized learning and teaching.
Tens of thousands of questions from a variety of sources including the NY Regents, state assessments, NAEP and TIMSS, and three NC math contest organizations can be found at Problem-Attic.
All of the questions are in a database, organized by topic, and individually selectable and re-formattable. They can be inserted into all kinds of new documents: tests and quizzes, worksheets, flash cards, and slides.
Problem-Attic is a product of EducAide Software, which has a long history of developing question banks and other teaching/testing tools.
